On or about January 17, 2018, the employer did not ensure that each employee who walked on the warehouse rear dock area was protected from a 4-foot, 6-inch fall hazard to the concrete lower level below by the use of a guardrail system.

29 CFR 1910.157(c)(4): Portable fire extinguishers were not maintained in a fully charged and operable condition:  a. On or about January 17, 2018, in the Metal Shop, where welding and cutting operations were conducted, all six (6) portable fire extinguishers had been used during a previous fire and were not in a fully charged or operable condition, thereby exposing employees to entrapment, smoke inhalation and burn hazards in the event of another fire.  b. On or about January 17, 2018, in the Warehouse Production area a portable fire extinguisher's pressure gauge indicated that the fire extinguisher was in an UNDERCHARGED condition and was not in a fully charged or operable condition, thereby exposing an employee to entrapment, smoke inhalation and burn hazards in the event of a fire.
