Employer failed to correctly evaluate the space for a hazardous atmosphere.

The only inspections that were found to be performed were at the start of the first shift on weekdays. The strobe light on top of the roof was inoperable.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    Maintenance Shop - The work rest installed on the United 8-inch bench grinder was not properly adjusted to prevent the work from being jammed between the wheel and work rest which may cause wheel breakage. The work rest measured 1/4 inch away from the wheel on the right side.

Located in the Laboratory, there was an airhose that was used in part to supply air to a pneumatic needle gun. The airhose was attached to the hose fitting by a worm drive type clamp which was not designed for use on an airhose, thereby exposing the employees to the whipping action of the hose if the clamp were to come off.     b. Located in the Electroplating room, there was an airhose that was used in part to supply air to a pneumatic needle gun.
