29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Shell Department - The work rest installed on the JET 6-inch bench grinder was not properly adjusted to prevent the work from being jammed between the wheel and work rest which may cause wheel breakage. The work rest measured 3/4 inch away from the wheel on the right side.

The adjustable tongue measured 7/8 inch.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected. Wax Department Area - The airhose connected to a pneumatic needle gun was attached using a worm drive type clamp which was not designed for use on an airhose. Air pressure was 80 p.s.i. and employees were exposed to the whipping action of the hose if the clamp were to come off.
