29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).    On or about February 7, 2018, a Pacific Trail Mfg., Inc. Accu-Cut crosscut electrical disconnect located in the control panel was not functioning properly and would not isolate the machine as required for lockout tagout.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    On or about January 10, 2018, an employee operating the Pallet Repair Systems, Inc. pallet dismantler was not provided training in the company's energy control program when performing servicing and maintenance work on the equipment such as changing blades.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    (a)  On or about January 10, 2018, employees were exposed to crushed by hazards when reaching into an Automated Industrial Technologies Auto Stacker that had the guard removed.      (b)  On or about January 10, 2018, employees were exposed to struck by hazards from a pallet flipper on a Viking Turbo Model 505 pallet assembly machine located in the assembly department that had expanded metal guards with sections cut out.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.  (a)  On or about January 10, 2018, a Cornell chop saw located in the hardwoods department had broken expanded metal guards which were not securely affixed to the machine creating sharp edges which resulted in laceration hazards to employees.  (b)  On or about January 10, 2018, a Viking Turbo Model 505 pallet assembly machine located in the assembly department had expanded metal guards with sections cut out creating sharp edges which resulted in laceration hazards to employees.

29 CFR 1910.213(a)(9): All belts, pulleys, gears, shafts, and moving parts shall be guarded in accordance with the specific requirements of 1910.219.    On or about January 10, 2018, a Cornell chop saw located in the hardwoods department did not have the drive belt and pulley under seven feet from the floor guarded as required in 29 CFR 1910.219(d)(1) and (e)(3)(iii).  The guard in placed extended approximately 6 feet 5.5 inches from the floor.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for fixed wiring of a structure; where run through holes in walls, ceilings, or floors; where run through doorways, windows, or similar openings; where attached to building surfaces; where concealed behind building walls, ceilings, or floors; or where installed in raceways, except as otherwise permitted in this subpart.      (a)  On or about January 10, 2018, a flexible extension cord located near the knotcher and re-saw was used as fixed wiring to power a fan and attached with zip ties to the building structure.    (b)  On or about January 10, 2018, a flexible extension cord located near the pallet dismantler was used as fixed wiring to power fans and was concealed behind equipment.
