29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) At the Davenport Florida facility, employees are exposed to struck by, crushing hazards when entering into the #3 Conveyor Belts and Palletizer machine SN 203E6553D4, to clear jams, while using the access barrier door Interlock as a lockout tag out method and not utilizing lockout tag procedures to de-energizes the palletizer.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees from hazards created by rotating parts: (a) For employees at the Davenport, Florida facility, inside of the Conveying Industries palletizer SN 203E6553D4, the safety limit switches were not operational and did not maintain the pusher arm in a stopped position when the access door was opened while the palletizer was energized on or about 1/11/2018.
