29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered the energy control procedure:    a)  Future Foam, Inc., 11475 E. 40th Ave., Denver, CO  80239:  On and before December 18, 2017, the employer did not ensure procedures were developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered the energy control procedure.  The employer had not developed specific procedures for the control of hazardous energy for equipment covered under this standard such as, but not limited to, maintenance and changing out saw blades.  An employee came into contact with a horizontal saw blade when spraying lubricant on the blade.  This condition exposed employee to amputation hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  Future Foam, Inc., 11475 E. 40th Ave., Denver, CO  80239:  On and before December 18, 2017 the employer did not ensure that one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation and rotating parts.  The employer operated a horizontal band saw that included a moving tables where foam was placed.  The employer did not minimize exposure to the saw blade and table.  The non-working parts of the blade were not guarded.  The working part of the blade was not shielded to prevent employee contact.  In addition, the moving table was not guarded or sectioned off to prevent employees from getting struck by the moving table.  These conditions exposed employees to amputation hazards and to getting struck by the moving table.
