29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.  a. On or about January 18, 2018, in the main shop, an open-sided platform was not protected by a guardrail along multiple sections of the storage area located along the north side of the main shop there by exposing employees to a fall hazard of more than eight feet.  b. On or about January 18, 2018, in the main shop, a platform was not protected by a standard guardrail system in that lattice was used in place of a midrail along a portion of the storage area there by exposing employees to a fall hazard of more than eight feet.  c. On or about January 18, 2018, in the main shop, an open-sided platform used to transition from the storage area to the top of work cells was not protected by a guardrail  there by exposing employees to a fall hazard of more than eight feet.  d. On or about January 18, 2018, in the main shop, an open-sided platform used for storage above individual work cells #5 and #6 was not protected by a guardrail  there by exposing employees to a fall hazard of more than eight feet.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails as follows:   On or about January 18, 2018, in the main shop storage area, an open stairway with 15 risers leading to the storage area was not provided with a stair rail system that included a midrail thereby exposing employees to a fall hazard.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:    a. On or about January 18, 2018, in the main shop storage area, near the fire exit door, ten compressed gas cylinders were not properly secured or supported to prevent the cylinder from falling, thereby exposing employees to a struck by hazard.   b. On or about January 18, 2018, in the main shop storage area, near work cell #5, one compressed Argon gas cylinder was not properly secured or supported to prevent the cylinder from falling, thereby exposing employees to a struck by hazard.

On or about January 18, 2018, in the main shop, the employer did not ensure eight openings in circuit breaker panel were covered, there by exposing employees to a shock and or fire hazard. Additionally, three opening (knockouts) on the left side of the panel were not effectively closed.   b. On or about January 18, 2018, in the main shop, one unused opening (knockout) in a  four way electrical box was covered, there by exposing employees to a shock and or fire hazard.

On or about January 18, 2018, in the main shop, the employer used flexible cable as a substitute for fixed wiring in that a flexible cord was used to power a four way outlet which provided power to an sandblaster fluorescent light there by exposing employees to an electrical hazard.   b. On or about January 18, 2018, in the main shop, the employer used flexible cable as a substitute for fixed wiring in that a flexible cord was used to power various pieces of equipment such as but not limited to the Lincoln electric Tig Welders there by exposing employees to an electrical hazard.
