29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) Roof of the building - On or about January 19, 2018 an employee was exposed to potential permanent eye injuries while cutting metal. This employee operated Hitachi G12SS2 grinder without wearing eye protection.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. (a) Roof of the building - On or about January 19, 2018 five employees were exposed to a fall hazard without fall protection 15 feet 7 inches above concrete. RK WALLACE CONSTRUCTION DBA WALLACE STEEL ERECTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1926.760(A)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1155469 CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON OCTOBER 12, 2016 WITH A FINAL ORDER DATE OF NOVEMBER 7, 2016 WITH RESPECT TO A WORKPLACE LOCATED AT 5805 STATE HWY 25, FLOWOOD, MISSISSIPPI 39232.
