29 CFR 1910.178(q)(7): Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  (a)   Construction site - On or about January 19, 2018 three employees were exposed to a potential stuck-by hazard due to a forklift not having a seatbelt installed.  Employees were handling metal decking in front of the forklift.

29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.  (a)  Construction site - On or about January 19, 2018 three employees were exposed to a potential stuck-by hazard due to the forklift operator not wearing his seatbelt.

(a) Lower side of the roof - On or about January 19, 2018 three employees were exposed to potential head injuries while lifting and carrying metal decking overhead with no hardhats.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems.  (a)  Roof - On or about January 19, 2018 three employees were exposed to a fall hazard while installing metal decking on a roof 15 feet 2 inches to 16 feet above asphalt with no fall protection. One employee leaned over the lower edge of the roof.
