29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction located on Lot #5, Roof: On or about 01/19/2018, and at times prior, the employer exposed employees to struck-by hazards while operating pneumatic framing nail guns at 120 PSI, in that employees were not provided with protective eyewear while engaged in framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #5, Roof: On or about 01/19/2019, and at times prior, the employer exposed employees to fall hazards of 22-feet, 4-inches to the lower level, in that a fall protection system was not provided to employees engaged in plywood sheathing installation on a 10/12 pitched roof.

29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use:  (a) New residential construction located at Lot #5, First floor inside structure: On or about 01/19/2018, and at times prior, the employer exposed employees to fall hazards of 10-feet, 8-inches to the lower level, in that a job made ladder provided did not have uniform rungs/cleats as the space between the rungs were spaced between 5-inches and up to 33-inches apart. The employer failed to provide safe access to the upper level.
