29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about January 23, 2018, and times prior thereto, at the jobsite located at the 100 Block of East 4th Street in North Little Rock AR, an employee was observed using a Hitachi pneumatic nail gun without the use of eye protection.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about January 23, 2018, and times prior thereto, at the jobsite located at 100 Block of East 4th Street, North Little Rock AR, the employer did not provide proper access to the elevated work area where employees installed roof trusses.  The employees used wood-frame bracing components access the work areas.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The Murillo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1211206, citation number 1, item number 1 and was affirmed as a final order on May 2, 2017, with respect to a workplace located at 7224 S. Elwood Avenue, Tulsa, OK 74132.  The Murillo Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 175727, citation number 1, item number 1 and was affirmed as a final order on March 1, 2017, with respect to a workplace located at 8748 N. Mingo Rd, Owasso, OK 74055.  On or about January 23, 2018, and times prior thereto, at the jobsite located at 100 Block of East 4th street, North Little Rock, AR each employee observed working on wood trusses, did not have proper fall protection.
