29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel:    a)  On or about January 24, 2016 in the production shop-bay #3, where an employee was exposed to struck-by hazards when using a 5 inch Dewalt portable grinder serial # 023899 that did not have a guard on it.    b)  On or about January 24, 2018, in the production shop-bay #13, where employees were exposed to struck-by hazards when using a 5 inch Dewalt portable grinder serial # 023908 that did not have a guard on it.    c)  On or about January 24, 2018, in the production shop-bay#16, where employees were exposed to struck-by hazards when using a 5 inch Dewalt portable grinder serial # 19345 that did not have a guard on it.    d)  On or about January 24, 2018, in the production shop-bay#22, where employees were exposed to struck-by hazards when using a 5 inch Dewalt portable grinder serial # 023926 that did not have a guard on it.
