OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by the load should the components of the overhead hoist/bridge cranes fail:  a)  General Machine Shop, OKK 860 vertical milling center, on or about 1/25/18:   A Coffing overhead hoist rated for 500 lbs. was attached to a jib crane rated for 1/2 ton and annual inspections were not being conducted.  b)  General Machine Shop, OKK 800 horizontal milling center, on or about 1/25/18:  A Coffing overhead hoist rated for 500 lbs. was attached to a jib crane rated for 1/2 ton and annual inspections of the lifting equipment were not being conducted.  c)  General Machine Shop, OKK 800 horizontal milling center, on or about 1/25/18:   A CM Shop Star overhead hoist rated for 250 lbs. was attached to a jib crane rated for 1/2 ton and annual inspections of the lifting equipment were not being conducted.  d)  General Machine Shop, OKK 800 horizontal milling center, on or about 1/25/18:  A CM Shop Star overhead hoist rated for 250 lbs. was attached to a jib crane rated for 1/2 ton and annual inspections of the lifting equipment were not being conducted.  e)  General Machine Shop, OKK HM800 horizontal milling center, on or about 1/25/18:  A Budgit overhead hoist rated for 1/4 ton was attached to a jib crane rated for 750 lbs. and annual inspections  of the lifting equipment were not being conducted.  f)  General Machine Shop, OKK MCV 860, on or about 1/25/18:  A Budgit overhead hoist rated for 1/4 ton was attached to a jib crane rated for one ton and annual inspections of the lifting equipment were not being conducted.  Abatement certification must be submitted for these items.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  a)  At the establishment, on or about 1/25/18:  Annual training on the fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided on an annual basis in that the last training was conducted on 4/10/14.   The Fulton Tool Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 963901, citation number 1, item number 1 and was affirmed as a final order on 4/16/14, with respect to a workplace located at 802 West Broadway, Fulton, NY 13069.
