29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished:    Covestro, LLC, 8 FairviewWay, South Deerfield, Massachusetts;  On or before December 27, 2017 employees did not verify that the line 11 heaters were deenergized as part of the energy control procedures, prior to performing work on or near the heaters.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    Covestro, LLC, 8 Fairview Way, South Deerfield, Massachusetts, On or before January 25, 2018 the employer did not have a program in place to identify damaged cords, plugs, and insulation on cord and plug equipment:    a. Damaged cord with cut in the insulation of greater than an inch in length.  b. Relocatable power taps being supported by the plugs of equipment plugged into them.  c. Minimum bend radii of cords for lights and portable equipment crimping the cords  d. Insulation on portable cords pulled away from the plugs due to insufficient strain relief  e. Crushed plug on a portable tilter

29 CFR 1910.137(c)(2)(xii): The employer shall certify that equipment has been tested in accordance with the requirements of paragraphs (c)(2)(iv), (c)(2)(vii)(D), (c)(2)(viii), (c)(2)(ix), and (c)(2)(xi) of this section. The certification shall identify the equipment that passed the test and the date it was tested and shall be made available upon request to the Assistant Secretary for Occupational Safety and Health and to employees or their authorized representatives.                                                                                                                                                                 Note to paragraph (c)(2)(xii): Marking equipment with, and entering onto logs, the results of the tests and the dates of testing are two acceptable means of meeting the certification requirement.  Covestro, LLC, 8 Fairview Way, South Deerfield, Massachusetts; On and before January 25, 2018 employees were using electrically rated gloves that did not have the test date marked on the gloves.
