29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval:   (a) Thunderbolt Builders Inc. located at 3502 Big Ben Rd. Fort Collins, CO 80526:  On and before January 19, 2018, the creating employer did not ensure that modifications or additions affecting the safe operation of the equipment were made with the manufacturer's written approval.  A job made platform was used as an attachment on a leased Genie rough terrain forklift model GTH-844.  The employer built a platform which attached to the forks for the framer's use.  The capacity, operation, and maintenance instruction plates, tags, and decals for the forklift were not changed accordingly to reflect the weight of the platform.  This exposed employees to struck-by hazards.
