29 CFR  1926.1053(b)(6):  Ladders were not used only on stable and level surfaces unless secured to prevent accidental displacement:  a)  Thunderbolt Builders, Inc., 68 Chipeta Way, Brighton, Colorado, 80601:  On and before April 7, 2021, the exposing employer did not ensure that ladders were used only on stable and level surfaces unless secured to prevent accidental displacement.  An employee placed an extension ladder on several pieces of oriented strand board (OSB) while stapling off the roof edge on a new construction, residential home. The employee fell 20 feet from the ladder and sustained serious injuries.  This condition exposed employee to fall hazards.

29 CFR  1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder shall be secured at its top to a rigid support that will not deflect:  a)  Thunderbolt Builders, Inc., 68 Chipeta Way, Brighton, Colorado, 80601:  On and before April 7, 2021, the exposing employer did not secure the portable extension ladder when being used to access to an upper landing surface to finish the nailing pattern on the roof edge. An employee placed an extension ladder on several pieces of oriented strand board (OSB) while stapling off the roof edge on a new construction, residential home. The employee fell 20 feet from the ladder.  This condition exposed employee to fall hazards.
