29 CFR 1904.10(b)(4): Incidents of recordable Standard Threshold Shifts were not recorded on the OSHA 300 Log of Injury and Illness when the affected employee(s) were not retested within 30 days of the first test: a) Employees #10556, 10610, 10426 and 10416 were exposed to noise and suffered recordable standard threshold shifts as documented on July 27, 2017. The employees did not have the follow-up testing, and the incidents were not recorded on the 2017 OSHA 300 Log. Observed on January 26, 2018. ABATEMENT CERTIFICATION REQUIRED
