29 CFR 1926.1053(a)(1)(ii): A job-made ladder was not built in conformance with the applicable provisions of Appendix A:  a) On January 26, 2018, employees used a job-made ladder to access the second floor of a building under construction from the exterior of the building. The ladder was not built in conformance with Appendix A which references American National Standards Institute (ANSI), A 14.4-1979-Safety Requirements for Job-Made Ladders.
