29 CFR 1910.119(h)(3)(iii): Contract employer did not document that each contract employee received and understood the training required by this paragraph to include a record which contained the identity of the contract employee, date of the training, and means used to verify understanding of the training; b) On or about 01/02/18 at a PSM covered facility; employees were exposed to burns caused by contact with ammonia liquid and vapors during the testing of the safety shut down system of the System #1 intercoolers when the contract employer did not document their employees' training required by this paragraph, such as but not limited to, opening of process equipment, i.e. line breaking and control of hazardous energy (LOTO). ABATEMENT DOCUMENTATION REQUIRED
