29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Tube Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using an OmniTurn CNC Machine (Model: GT-75, SN: 4817 PTI) without a safety door installed. b) Tube Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Buffalo Machines 5 speed heavy duty drill press (Model: DSM-5, SN: Not Available) without a chuck guard installed. c) Swage Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Buffalo Machines drill press (Cat. No: M3534, SN: Not Available) without a chuck guard installed. d) Machine Shop Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Taft Peirce surface grinding machine (Model 9602-1000, SN: 220-9812) without a guard installed. e) Machine Shop Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Craftsman 15 inch drill press (SN not available) without a chuck guard installed. f) Machine Shop Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Bridgeport drill press (SN: J40305) without a chuck guard installed. g) TC Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Roll-In Saw Company band saw (No. 2) without a chuck guard installed. h) TC Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Walker Turner Company drill press (No. 6) without a chuck guard installed. i) TC Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Leeson tapping machine (Model A4C17DJ3A) without having the rotating belts guarded. j) Silicone Rubber Working Area: On or about January 30, 2018, and at times prior thereto, employees were exposed to caught-in hazards while using a Coil Winding Machine (Model Not Available) without having a guard installed.
