29 CFR 1910.37(a)(3): Exit route(s) were not kept free and unobstructed: a. 91 Appalachian Trail Drive, Cleveland, GA (Shop Area) - Employees were exposed to entrapment hazard. One of the designated marked, and identified emergency exit door in the shop area was blocked with a large heavy bin where the employees are discarding their excess wood materials. The large bin was placed directly in front of the emergency exit door, on or about January 31, 2018.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a. 91 Appalachian Trail Drive, Cleveland, GA - Maintenance employees were exposed to sudden energization or start-up of multi-energy source equipment/machinery such as, the moulder machines. The company's written lockout/tagout specific procedures were found with the following deficiencies: The energy control procedures did not include the set-up procedures for the moulder machines when performing tool adjustment such as, changing out the head/blade on the moulders as part of normal operation, on or about January 31, 2018.

The machine operator and/or laborer employee were working in close proximity to the moulder machine #4, while the 4X10 metal cover which serves as a guard, was missing and/or removed from the top section of the head/cutter blade partially exposing the rotating blade, on or about January 29, 2018.
