On or about January 30, 2018, the Process Hazard Analysis PETN Production, dated January 2017, did not address all of the hazards of the process for B2: Nitration/Nitrator RB-111 & RB-112 including, but not limited to, the following: deviation of high temperature (liquid phase), its causes and potential effects. b. On or about January 30, 2018, the Process Hazard Analysis PETN Production, dated January 2017, did not address all of the hazards of the process for B2: Nitration/Nitrator RB-111 & RB-112 including, but not limited to, the following: deviation of high temperature (vapor phase), its causes and potential effects. c. On or about January 30, 2018, the Process Hazard Analysis PETN Production, dated January 2017, did not address all of the hazards of the process for B2: Nitration/Nitrator RB-111 & RB-112 including, but not limited to, the following: potential effects for the deviation of high level such as high temperature in vapor phase due to loss of mixing cyclone (reaction at liquid vapor interface).

29 CFR 1910.119(e)(3): The process hazard analysis shall address: 29 CFR 1910.119(e)(3)(iii): Engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases. (Acceptable detection methods might include process monitoring and control instrumentation with alarms, and detection hardware such as hydrocarbon sensors.). a. On or about January 30, 2018, the Process Hazard Analysis PETN Production, dated January 2017, did not address all of the engineering and administrative controls/safeguards for the hazards associated with high temperature (liquid phase) in B2: Nitration/Nitrator RB-111 & RB-112. b. On or about January 30, 2018, the Process Hazard Analysis PETN Production, dated January 2017, did not address all of the engineering and administrative controls/safeguards for the hazards associated with high temperature (vapor phase) in B2: Nitration/Nitrator RB-111 & RB-112. c.
