29 CFR 1910.178(l)(3)(ii)(I): Powered industrial truck operators did not receive initial training on other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation: (a) On or about 1/25/2018, Grinder Stockpile, training was not provided to forklift operators that were exposed to fall hazards while required to dismount their forklifts on a wet slippery surface in order to unlatch a self-dumping hopper.
