29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) Site 79 & 32 in Lancaster, PA - Employees were exposed to possible trench collapse hazards while performing pipeline construction through Southern Lancaster County PA location 79 and 32. The employer failed to instruct/train each employee in the recognition and avoidance of unsafe trench and appropriate soil classification, on or about February 1, 2018. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

(a) Site 79 & 32 in Lancaster, PA - Employees entered a trench 13 feet deep that was not provided with adequate sloping and benching system or a supported system, shield system, or other protective systems thereby exposing employees to a trench collapsed, on or about February 1, 2018. No abatement certification or documentation required.
