29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder: a - 2 & 4 Sablow Drive, Lakewood NJ: A company worksite employee was exposed to fall hazards when not having at least one hand grasping the ladder as they were progressing up the ladder. At the time of the exposure, this employee was carrying tools and equipment with his one arm and hand therefore preventing him from always maintaining three points of contact when progressing up the ladder. Condition observed on or about 02/05/18. 29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a - 2 & 4 Sablow Drive, Lakewood NJ: Company worksite employees were exposed to fall hazards when using an aluminum extension ladder that contained missing and visibly damaged components and was not set on a stable surface. At the time of the worksite inspection, these employees had not received any ladder hazard recognition training or training on procedures to be followed to minimize these hazards. These condition were noted on or about 02/05/18. Giant Builders, Inc. was previously cited for a Repeat violation of this Occupational Safety & Health Standard or its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 1258354, Citation Number 2, Item Number 1 and was confirmed as a final order on 12/01/18, with respect to a workplace located at 67 West Street, Spring Valley NY. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
