29 CFR 1915.89(c)(1): The employer did not ensure that, before any authorized employee performs servicing when energization or startup, or the release of hazardous energy, may occur, all energy sources were identified and isolated, and the machinery, equipment, or system was rendered inoperative.  (a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and death while connecting or disconnecting wiring in the circuit breakers of the temporary load centers. The employer did not ensure that the energy sources were isolated prior to working in the circuit breaker enclosures (GE TE100R Mod. 8 type 3R), with dead fronts that were intentionally removed.

(a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and death while connecting or disconnecting wiring in the circuit breakers of the temporary load centers. (a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and death while connecting or disconnecting wiring in the circuit breakers of the temporary load centers. The employees were working in the GE TE100R Mod. 8 type 3R panel enclosures with energized 100A, 480VAC and 30A, 480VAC circuit breakers without arc flash protective clothing and/or equipment.

(a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and death while connecting or disconnecting wiring in the circuit breakers of the temporary load centers. The employees were working in energized circuit breaker enclosures (GE TE100R Mod. 8 type 3R) with dead fronts that were intentionally removed and not re-installed per the listing and labeling from Underwriters Laboratories (UL).

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  (a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, the employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and even death from contact with live conductors through the unused openings on General Electric (GE) circuit breaker enclosures TE 100R MOD 9 Type 3R.   Vigor was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.305(b)(1)(ii), which was contained in OSHA inspection number 1138205, citation number 1, item number 4a and was affirmed as a final order on 05/27/2016, with respect to a workplace located at Vigor Alaska, LLC. Mile 7 Nash Rd. Seward, AK. 99664.
