On or about February 6, 2018, at the site located at 762 Waldron Street, Columbus, Ohio, employees were operating nail guns to install framing without eye protection, thereby exposing the employees to potential eye injuries.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about February 6, 2018, at the site located at 762 Waldron Street, Columbus, Ohio, employees were performing framing work on a three story residential apartment building while standing on the third story floor level at approximately 20 feet in height with no fall protection, thereby exposing the employees to a fall hazard.
