29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not required to wear appropriate goggles, to protect them from the rays when noncombustible or flameproof screens or shields were not used: (a) Weld Shop, on or about February 21, 2018 or prior thereto: employees working in areas of welding operations, were not protected from flash burns from exposure to ultraviolet radiation. (b) Plasma Jet, on or about February 21, 2018 or prior thereto: employees working in areas adjacent to plasma cutting operations, were not protected from flash burns from exposure to ultraviolet radiation. Note: Abatement certification is required for this item.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) On February 13, 2018 or prior thereto: the employer exposed employees to potential hazardous energies in that employees were allowed to service and perform preventative maintenance without equipment specific procedures.
