On about 2/06/2017, US Pack Logistics, LLC, located at 4550 Gustine Ave. in St. Louis, MO, had not ensured that each powered industrial truck (PIT) operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training. An evaluation of the operators performance had not been conducted on any of the PIT operators onsite.

29 CFR 1910.178(p)(1): A powered industrial truck was found to be in need of repair, defective, or in any way unsafe, the truck and was not be taken out of service until it had been restored to safe operating condition. On about 2/06/2017, employees at US Pack Logistics, LLC were operating defective powered industrial trucks in need of repair that were not removed from service until they had been restored to safe operating condition. The following defective powered industrial trucks were in use onsite: a) Komatsu PIT, Model FG25ST-16, Serial # A235067: The LPG tank hold down strap was damaged and not attached to prevent displacement and the brake pedal pad was missing. b) Toyota Electric Truck, Model 6HBE30, Serial # 6HBE30-27965: The operator controls on the handle was missing the bezel and buttons for the electrical switches.
