29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): On or about 06/21/2019, employees at US Pack Logistics, LLC were operating defective powered industrial trucks in need of repair that were not removed from service until they had been restored to safe operating condition. The following defective powered industrial trucks were in use onsite: 1) Landall PIT, Model #B4048IC - 1800, Serial #B4048IC389802667: One of the rear tires was severely damaged due to missing chunks of the tire. 2) Landall PIT, Model #B30-42-605B-04067, Serial #B30-42E1800: The back-up alarm was inoperable and one of the lights was out.

29 CFR 1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, and maintained in a safe condition; On or about 06/21/2019, at the roll up doorway entrance from the main building to the shipping dock, there was a one inch gap between the two walking-working surfaces, exposing employees to potential tripping hazards.
