On 02/06/2018, at 1305 Old Fountain Rd, Lawrenceville, GA 30043, employees were exposed to fall hazards when performing brick mason activities to a height of approximately 12 feet on a scaffold that was not fully planked or decked.

On 02/06/2018, at 1305 Old Fountain Rd, Lawrenceville, GA 30043, employees were exposed to fall hazards when working on a scaffold to a height of approximately 12 feet which the ends of the platforms were extended for 40 inches over upright.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  On 02/06/2018, at 1305 Old Fountain Rd, Lawrenceville, GA 30043, employees were exposed to fall hazards when working on a scaffold to a height of approximately 12 feet without a ladder to have direct access to the scaffold and using the cross braces as a mean to access and egress from the scaffold.
