29 CFR 1926.1051(a):  Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a)  5th Floor - Employees conducting framing operations were not provided proper access to the top of the wood joists where there was an approximate 5 foot break in elevation, on or about February 12, 2018.    No abatement certification or documentation required.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a) 5th Floor - An employee was observed without eye protection while using a pneumatic nail gun to attach sheathing to roof joists and was exposed to flying wood chips and ricochet nails, on or about February 12, 2018.    No abatement certification or documentation required.    Keystone Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 969110, citation number 1, item number 1, and was affirmed as a final order on August 4, 2014, with respect to a workplace located at 5502 Main Street, East Petersburg PA 17520.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a) 5th Floor - An employee working on the top floor of a hotel building donned a full-body harness as part of personal fall arrest system but was not attached to an anchor point or otherwise protected from a 48 foot fall hazard to the ground below, on or about February 12, 2018.    No abatement certification or documentation required.    Keystone Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 969110, citation number 2, item number 1, and was affirmed as a final order on August 4, 2014, with respect to a workplace located at 5502 Main Street, East Petersburg PA 17520.
