29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. On Feb. 12, 2018, located on the north side of a commercial roof structure, an employee was operating a JLG G5-18A Rough Terrain Forklift that had not received an evaluation of the operator's performance in the workplace as required by this subpart.

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers. On Feb. 12, 2018, there were no frequent and regular inspections being performed by a competent person to ensure the jobsite, materials and equipment such as warning line systems, fall protection devices and/or personal fall arrest systems were being maintained and utilized in a safe condition.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted. On Feb. 12, 2018, located on the east and south sides of a commercial roof structure, employees were observed performing roofing work on a low-slope roof while not utilizing a fall protection system in that a warning line system was not erected around all sides of the work area. Potential fall heights were measured to be greater than 15 feet above the ground below.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: On Feb. 12, 2018, located on the north side of a commercial roof structure, an employee was attached to a Guardian Temper Anchor with D-Ring that was not capable of supporting 5,000 pounds in that an insufficient amount of nails was used to support a personal fall arrest system which maintains a safety factor of two. Potential fall height measured to be greater than 15 feet above the ground below.
