29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket).  450 Shed, on or about 08 February 2018 and times prior thereto; a 50-foot black heavy duty extension cord had damage to the outer protective jacket. This condition exposed employees to an electrical shock hazard.

Maintenance shop; on or about 08 February 2018 and times prior thereto, the maximum safe load limits was not conspicuously posted on the mezzanine. This condition exposed employees to a struck by hazard.

29 CFR 1917.151(b)(3): Fixed machinery was not secured to prevent shifting.  Maintenance shop, on or about 08 February 2018 and times prior thereto, two instances were noted where floor drill presses where not securely anchored:       a. Welding and fabrication area; a 20-inch, Westward Drill Press, 12-Speed, Model 1KEN7, Serial number unknown was not securely anchored to prevent movement        b. Southwest bay; a 15-inch Jet Floor Drill Press, Model J-2500, Serial Number 17021687 was not securely anchored to prevent movement.  This condition exposed employees to a struck by hazard.

29 CFR 1917.156(a)(9): Liquid fuel dispensing devices, such as pumps, were not mounted on a concrete island or otherwise protected against collision damage.  Yard area adjacent to maintenance shop; on or about 08 February 2018 and times prior thereto, a 500-gallon gasoline dispensing station was not protected from vehicular traffic. Dispensing station is located adjacent to a moderate vehicular traffic area. This condition exposed employees to fire and explosion hazards.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed.  Maintenance shop and JSA room; on or about 08 February 2018 and time prior thereto, four instances were noted where electrical opening were not effectively closed.       a. Maintenance shop northeast area, a four-place energized outlet box was missing a knockout plug on the left hand side.       b. Maintenance shop southwest area, a two-place energized outlet with a rocker switch was missing the knockout plug on the top of the outlet box.       c. JSA room, south wall two energized electrical outlets were missing the faceplates.  This condition exposed employees to an electrical shock hazard.   Offshore Systems Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(b)(1)(ii) which was contained in OSHA inspection number 1041808, citation number 1, item number 3 and was affirmed as a final order on 20 April 2015, with respect to a workplace located at OSI Dock, Captains Bay Road in Dutch Harbor, AK.

29 CFR 1910.303(b)(7)(iv): Electrical equipment had damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating.  450 Shed, adjacent to the man door leading to the dock; on or about 08 February 2018 and times prior thereto; three energized rocker switches were damaged. Switches labeled South, Center, and North were broken. This condition exposed employees to damaged electrical equipment.

29 CFR 1917.122(a): Employee exits were not clearly marked.  North area of maintenance shop; on or about 08 February and time prior thereto, the employer did not ensure that exit signs were posted along the exit route. The door way leading to the north exit route was not marked. This condition exposed employees to delays in exiting the building causing smoke inhalation and fire hazards.
