29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: (a) Weather Deck: On or about February 8, 2018 and times prior thereto, the Trashpacker 7000 was being cycled with the door open and employee pushing on the garbage. This exposed employee to a caught-in hazard from operating the equipment in this condition.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): (a) Bader Tech shop: On or about February 8, 2018 and times prior thereto, the Jet 6 inch bench grinder was cut to the conductor. This exposed employees to minor shock/burn hazards.
