29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:    (a)  JD Building Services, LLC on or about February 13, 2018, at the address of 521 Louisiana Ave. Perrysburg, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment.  Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) JD Building Services, LLC on or about February 13, 2018, at the address of 521 Louisiana Ave. Perrysburg, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing work tasks from the elevated roof deck of a commercial structure.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a)  JD Building Services, LLC on or about February 13, 2018, at the address of 521 Louisiana Ave. Perrysburg, OH: The employer did not ensure employees tasked with the removal of structural material from atop a wood framed commercial structure were adequately protected from falls. Employees cutting and rigging structural material from the elevated surface without the benefit of fall protection were exposed to an approximate 10 ft. fall to the ground below.
