29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems.  A) JD Building Services, LLC on or about April 12, 2018 at the address of 9328 Bowan Farms Lane, Sylvania, OH: The employer did not ensure employees were protected from falls while installing OSB sheeting to a residential type structure.  The employees were exposed to a potential 20 foot fall to the level below while working from the second level of a 7/12 pitch roof.          B) JD Building Services, LLC on or about April 12, 2018 at the address of 9328 Bowan Farms Lane, Sylvania, OH: The employer did not ensure employees were protected from falls while installing OSB sheeting to a residential type structure.  The employees were exposed to a potential 10 foot fall to the level below while working from the first level of a 5/12 pitch roof.           The employer JD Building Services, LLC was previously cited for a violation of an equivalent Occupational Safety and Health standard 1926.501(b)(1), which was contained in OSHA inspection number 1294496, citation number 1, item number 2 and was affirmed as a final order on March 14, 2018, with respect to a workplace located at 521 Louisiana Avenue, Perrysburg, Ohio.
