The workers were exposed to an electrical hazard.

29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket): JOB SITE: Hwy 160, PO BOX 163, Fairdealing, MO 63939, on or about 2/12/2018, a Ridgid Router, MOD: R2401, portable cord and plug connected equipment with damages to the outer jacket or insulation was being used in the production area. The extension cord was wrapped in numerous locations throughout the cord with black electrical tape to seal the damages to the outer jacket of the cord. The workers were exposed to an electrical hazard.
