An employee was progressing up a ladder with an object that may cause the employee to lose balance and fall.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    WORKSITE:  This violation was most recently observed on 2/13/2018 at a worksite at 98 Neptune Avenue, New London, CT 06320 where the side rails of a 24' Werner aluminum extension ladder was used for access to a flat pitched roof, approximately 18 feet above the ground, were not extended at least three (3) feet above the roof.     Connecticut Restoration, INC was previously cited for this practice, as described below.     Connecticut Restoration, INC was previously cited for a violation of this occupational safety and health standard {29 CFR 1926.1053 (b)(1)}, which was contained in OSHA inspection (961482), citation number 2, item number 2, affirmed as a final order on 5/08/2014, with respect to a workplace located at 45 Huntington Street, New London, CT 06320     Connecticut Restoration, INC was previously cited for a violation of this occupational safety and health standard {29 CFR 1926.1053 (b)(1)}, which was contained in OSHA inspection (916038), citation number 2, item number 1, affirmed as a final order on 7/23/2013, with respect to a workplace located at 387 North Main Street, Norwich, CT 06360.     Connecticut Restoration, INC was previously cited for a violation of this occupational safety and health standard {29 CFR 1926.1053 (b)(1)}, which was contained in OSHA inspection (897991), citation number 2, item number 2, affirmed as a final order on 5/13/2013, with respect to a workplace located at 189/191 Montauk Avenue, New London, CT 06320.   Connecticut Restoration, INC was previously cited for a violation of this occupational safety and health standard {29 CFR 1926.1053 (b)(1)}, which was contained in OSHA inspection (314407586), citation number 01, item number 04, affirmed as a final order on 02/25/2012, with respect to a workplace located at 78 Hempstead Street, New London, CT 06320.   To abate this violation, the employer must ensure that all ladders used to gain access to upper levels be placed in such a manner that their side rails extend at least 3 feet above the landing surface.    Pursuant to 29 CFR 1903.19 (d), the employer must provide documentation that this violation has been abated within 10 calendar days the employer receives the citation.

On or about 2/13/2018 employees at the Ocean Beach Park construction project were expos to approximately 30' falls.  Employees were engaged in carpentry activities withhout fall protection.  Connecticut Restoration, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 897991, citation number 02, item number 01 and was affirmed as a final order on 8/16/13, with respect to a workplace located at 189/191 Montauk Avenue, New London, CT 06320.
