29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about February 15, 2018, the employer failed to ensure that each authorized forklift operator was competent to safely operate powered industrial vehicles. Forklift operators had not been trained or evaluated. Employees were exposed to the hazards associated with unverified operators using powered industrial vehicles to load and unload materials in trucks and within the warehouse. There is no abatement of certification or documentation required for this item.
