29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: (a) Area near warehouse office and dock - On or about December 12, 2023, the employer failed to ensure that stored pallets were limited in height or secured against collapse. Forty (40) pallets, around 16 feet 8 inches in height, were stacked off-center without blocking means thereby exposing employees to falling object hazards that may result in crushing injuries or death. Abatement documentation or certification is not required of this item.

29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: (a) Warehouse - On or about December 12, 2023, the employer failed to ensure that forklift operators only handled stable or safely arranged loads. A forklift operator was permitted to transport two tall, palletized loads balanced one on top of the other thereby exposing employees to falling object hazards that may result in crushing injuries or death. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
