29 CFR 1910.147(d):  The established procedures for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):     On or about February 13, 2018, employees were exposed to the unexpected energization of the Mitsui Seiki VS5Q Vertical Spindle Machining Center, Model Number VS5Q, Serial Number 308, while conducting servicing and maintenance activities such as changing the bits/tooling.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.:  On or about February 13, 2018, employees working in the Tool Room at the Alliant Machine Tool, Model Number 1158-5V, Serial Number 1158-5V-1152, were exposed to embolism hazards because the compressed air used for cleaning purposes was measured at 104.9 psi.  The relief ports on the air gun nozzle were taped shut.  On or about February 13, 2018, employees working in the Tool Room at the Haas Tool Room Mill, Machine Number M1466, Model Number TM-1, Serial Number 48430, were exposed to embolism hazards because the compressed air used for cleaning purposes was measured at 64.4 psi.  The relief ports on the air gun nozzle were cut off.

29 CFR 1910.305(g)(1)(iv)(A):  Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  On or about May 10, 2018, employees working in the Press Area were exposed to electrical shock hazards because:  a.  the computer and monitor located behind the Cincinnati Press Brake, Machine Number M833, were plugged into a 6-outlet power strip which was plugged into an extension cord which was  plugged into another 6-outlet power strip which was plugged into a wall outlet.  b.  the two fans located between Cincinnati Press Brake, Machine Number M833, and Cincinnati Press Brake, Machine Number 1179, were each plugged into a 4-outlet electrical adapter which was plugged into an extension cord which was plugged into a 6-outlet power strip which was plugged into a wall outlet.  c.  the pedestal fan located between Cincinnati Press Brake, Machine Number 1179, and Cincinnati Press Brake, Machine Number 1167, was plugged into an extension cord which was plugged into another extension cord which was plugged into a wall outlet.  d.  the fluorescent light fixture located above the Amada America, Inc. Press Brake, Machine Number 1164, was plugged into an extension cord which was plugged into a wall outlet.
