29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed: Employees were exposed to caught-in hazards. Employees engaged in manufacturing activities at the Flattener, ID Number: CWP 26D11, within the Amada area in the main plant were exposed to caught-in and amputation hazards in that the employer had not identified additional hazards, through a written certification, when introducing new parts to be manufactured.
