29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about January 15, 2018, employees used a Hyster, serial D004V05776N, model S120XL, propane powered forklift to perform a change over on the 1040 hydraulic press. The employer had a die strapped and welded to the back of the forklift as additional counterweight in order to handle the weight of the die being changed out. The additional counterweight attached to the forklift was not approved by the forklifts manufacturer, thereby exposing employees to struck by and crushed by hazards.

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about February 22, 2018, the guide gears on the 168 hydraulic press located at Work Center 1060 were not adequately guarded, thereby exposing employees to a caught in hazard.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about February 22, 2018, the point of operation on the 168 hydraulic press located at Work Center 1060 was not adequately guarded. The press operator could remove hands from the two hand controls during the hazardous portion of the press cycle and the press would continue to close, thereby exposing employees to a caught in hazard.
