Main St. Merrill, WI: On February 15, 2018, employees were exposed to multiple hazards, including but not limited to, silica dust, PIV, and falls from scaffolding when the employer did not regularly inspect the jobsite to ensure safe work methods were being used, in that:    A) Scaffold erectors were working on levels that were not fully planked and where guardrails were not installed;    B) An RT forklift operator was not using his seatbelt;    C) Concrete block was being cut without dust suppression or respiratory protection;    D) Employees were not using appropriate PPE for their tasks.    E) Safe means of accessing the upper levels of the scaffold was not provided.
