29 CFR 1926.451(a)(6): Scaffolds designed by a qualified person were not constructed in accordance with that design: (a) Construction site located at 181 Carmichael Street, Hudson, WI; On or about August 28-29, 2018, fabricated frame scaffolding systems were not constructed in accordance with the scaffolding manufacturers design for issues including, but not limited to; bracing, access, planking, tying running scaffold, restraining freestanding towers, and fall protection on open ends/sides.
