Such construction shall comply with the test specifications set forth in Standard Methods of Fire Tests of Building Construction and Materials, NFPA 251-1969, to include openings to the room(s) be provided with approved self-closing fire doors and the room(s) be liquid-tight where the walls join the floor:  a) F/V Pacific Producer Main Deck Room 4, where approximately 80-100 gallons of flammable products including, but not limited to, gasoline, International Epoxy Paint Parts A and B, and Sunnyside Lacquer Thinner, were stored in an unventilated room constructed of wood that was not equipped with a self-closing fire door.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) F/V Pacific Producer, where employees were potentially exposed to inhalation hazards from an uncontrolled release of anhydrous ammonia from the charged refrigeration system that contained 6500 pounds of anhydrous ammonia.  A maintenance and repair program for the ammonia refrigeration system did not include:  1. Where the employer did not maintain a written system component inventory list.  2. Where the employer did not develop a written systems maintenance and inspection log to ensure the safety of the refrigerating systems.   3. Where corrosion protection was not uniform on all uninsulated piping and associated components such as flanges, the employer did not perform inspections to evaluate corrosion damage.   4. Where the employer did not post the location of protective, first aid and rescue equipment on notices within and external to the work areas containing anhydrous ammonia.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:   a) F/V Pacific Producer, where the employer did not develop a written hazard communication program. Employees utilized chemicals such as, but not limited to, International Paints Part A and Part B, Sunnyside lacquer thinner, Gloss spray paint, and gasoline. The charged refrigeration system located on board contained 6500 pounds of anhydrous ammonia.  The East West Seafoods, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(e)(1), which was contained in OSHA inspection number 956718, citation number 1, item number 2, and was affirmed as a final order on March 6, 2014, with respect to a workplace located at ISA Dock, Kodiak, AK.
