29 CFR 1915.73(e): When employees were working near the unguarded edges of decks of vessels afloat, they were not protected by personal flotation devices meeting the requirements of 29 CFR 1915.158(a):  a)  F/V Pacific Producer;  Main Deck, Port Side;  A section of the vessels midrail was missing above a cleat.  The missing guardrail section measured 43 inches high and 52 inches wide.   b)  F/V Pacific Producer;  Main Deck, Port Side;  A rope section of guardrail located near the compressed gas cylinder storage/ living quarters was loose with the top rail measuring 26.5 inches high.   c)  F/V Pacific Producer;  Main Deck, Port Side;  A chain guardrail measuring approximately 10 inches wide had come undone from a wooden storage unit (which stored SCBAs and concentrated sanitation liquid for the fish processing areas) and the metal guardrail of the vessel.   d)  F/V Pacific Producer;  Upper Deck, Port Side;  A 63 inch section of the vessels metal guard rail was missing a midrail.   e)  F/V Pacific Producer;  Upper Deck, Port Side;  The section of the vessel which stored the Guardian life rafts and Survival Suits was not guarded by an adequate removable guardrail system.   f)  F/V Pacific Producer;  Main Deck, Starboard Side, Bow Section;  A 71 inch wide section of the vessels metal guard rail was missing a midrail.   g)  F/V Pacific Producer;  Upper Deck, Starboard Side;  The section of the vessel which stored the Guardian life rafts and Survival Suits was not guarded by an adequate removable guardrail system.  h)  F/V Pacific Producer;  Upper Deck, Starboard Side;  A 54 inch wide section of the vessels metal guardrail underneath the crane was missing a top rail.

29 CFR 1915.74(a)(1): A gangway of not less than 20 inches walking surface of adequate strength, maintained in safe repair and safely secured was not used. If a gangway is not practical, a substantial straight ladder, extending at least 36 inches above the upper landing surface and adequately secured against shifting or slipping was not provided. When conditions are such that neither of gangway nor a straight ladder can be used, a Jacob's ladder meeting the requirements of (d)(1) and (2) were not used:  a)  F/V Pacific Producer moored at Shilshole Bay Marina, Seattle;  The employer did not provide safe access to the afloat vessel.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  a)  Main Deck, Port Side;  A nitrogen compressed gas cylinder was in storage and missing a cap.  East West Seafoods LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 956712, citation number 2, item number 1 and was affirmed as a final order on March 21, 2017, with respect to a workplace located at Kodiak, AK.

East West Seafoods LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 622260, citation number 1, item number 1 and was affirmed as a final order on March 21, 2017, with respect to a workplace located at Seattle, WA.
