29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: (a) On or about February 8, 2018, employees who were replacing the dies on Die Cast Machine #13 (DCM #13), were not required to: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy isolating device. (b) Employees wiping down the dies inside the die cast machines, were not required to: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy isolating device.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: (a) Employee training on the energy control program was inadequate, in that it did not include training employees on when and how to lock out the die cast machines prior to performing die changes and wiping down the dies.
