29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). Employees engaged in the operation of powered industrial truck reach stacker one were exposed to two instances of potential fall hazards while in the cab operating the machine. 1. Seat belt assembly was missing from the seat. 2. Right door handle in the in the cab would not latch.
